Acceptance Pak - Documents for Download

Please download the following documents and take the action described in each. 

• LIFE Immunization Requirements - This is a list of vaccinations that participants of LIFE must receive prior to the program.
• Doctor's Medical Form & Your Personal Health History Declaration - This document has two sections. One that participants fill out and the other that your physician will need to complete and fax back to us at: 1-480-275-3368
• Health Insurance -- There are two forms that you need to complete (ONLY the highlighted sections!) and fax back to LIFE at 1-480-275-3368  
  • The Insurance Health Declaration Form Only fill out the parts indicated in yellow highlight!
  • Health Insurance Policy "Proposal" - This is the application for a policy. Fill out only those parts highlighted in yellow on the form. The insurance covers you in Israel & India. 
• Visas for India and Israel -
  • India - If you have not already, please immediately email info@lifeprogram.org a digital photograph of your passport so we can prepare the letter needed to secure the special Visa needed for India.
  • Israel - Participants from most countries can enter Israel on a tourist visa, which is good for 3 months. Participants may need to renew their Israeli visa, at their own expense, during the program. Alternatively, participants can contact their Israeli consulate to receive a Masa visa. If this is your intention, download this document and present to the consulate. 
• Personal Information, Emergency Contact & Participant Declaration Form - This form includes everything we don't have from somewhere else -- from food, dietary and lifestyle preferences to emergency contact information, and language skills.
• The India Internship Interest Form - You should have received this by email, but it is here if needed.
• What to Bring - A packing list and information about LIFE in Israel and India and airline luggage weight limitations.
• When to Arrive/Where to Meet: The LIFE Program begins on Thursday, October 16 at 11am in Jerusalem.  Please make sure your flight (see below) can get you into Israel and to Jerusalem by 11am. Keep in mind that it may take an hour or so to exit the airport and about 1 ½ hours to drive to Jerusalem from the airport in a shared “Nesher” taxi-bus. Flight information,
• Flight Reservations – You should book your flight to Israel as soon as possible. Once you have made your flight reservations, please forward a copy of it to info@lifeprogram.org.
• Security Deposit - Note that participants will be expected to provide an undated security check in the amount of $1000 against apartment and program damages which will NOT be deposited and will be returned to you at the end of the program.
